Each individual who completes the assessment receives a personalized action plan identifying health risk factors, links to programs based on those risk factors, and next action steps for improving health.
- Integrated with a health screening – An HRA completed as part of a health screening can show a more complete picture of a participant’s health status because lifestyle information is evaluated along with clinical data, such as cholesterol levels, blood pressure, and blood glucose.

All HRA Results Can Feed Into Personal Care Management
Regardless of which HRA option is selected, data from HRA results can be used to trigger inclusion in MedCost Benefit Services’ Personal Care Management (PCM) nurse coaching program
. Participants who are identified as someone who may be at risk for developing a costly medical issue without intervention may be contacted by one of the PCM nurses to encourage enrollment in the program.
